Need Makkai & Pare pp 117-118

Hi,

Our (Ohio State U) library's copy of Makkai and Paré's  "Accessible Categories" is missing pages 117 and 118. I will be grateful if someone can send me a scan of those two pages. [The binding seems intact, and there does not seem to be any tear marks. I wonder if this was a "manufacturing defect" and afflicted a whole run.]
