Digging through the sources looking for clues, I discovered that I can run (nnmail-get-new-mail 'nnml 'nnml-save-nov nnml-directory) by hand in the *scratch* buffer and it does "the right thing". I'm more confused than ever, but at least I can read my mail now ;-)
