I've only seen this, since I started using the agent. I started using the agent on nnimap in mid-March (on March 19 to be exact), and have seen it twice since then. I usually run in plugged mode (ie. the agent cache is only used for speedup). I never encountered it when I used the agent regularily from 1999-2001. But then I mostly ran in unplugged mode. Maybe there's something there...?
