One of the big issues I've seen is that it can be repetative.

When I used nnmh (I think) gnus would read the whole thing three times
at startup.  Once was for finding new groups.  I forget why the third.

A single read through all of the spool would be enough....

Similarly, with nnimap it gets the details on each known group, then
does an lsub to get the full list of subscribed groups (as the imap
server understands the term) and gets the details on each group
returned by the lsub.  So every already-known group is detailed twice.

Again, it would be better to start with the lsub and iterate through all
of the groups once, noting for each whether gnus already knows about
that group.

Iterating only once would cut the startup time in half.
