That remains a mystery, I guess. Does M-g in the *Group* buffer fix
it? If not, what does nnmaildir-request-group put in the " *nntpd*"
buffer?

This has improved somewhat for me. When I first start Gnus, or after
g in the *Group* buffer, the counts are high (presumably due to
holes), but after some combination of M-g and entering the group, the
counts are right, and stay right at least until new mail arrives.
