Uhm? AFAICT in Emacs 20.3 it works as documented in `(elisp)Clickable
Text'. The original go at the Help mode hyperlinks used local-map
properties and it worked fine for mouse clicks. I'm not sure that
there was actually a good reason to change it.
[The hyperlinks probably should use widgets and be consistent with
custom, but rms wanted to postpone work on that.]
