Review: 16477 lmrc: move MFI definitions into their own headers

Hi,

I'm moving all the MFI-specific defines and structs into a separate set
of headers so that they can be shared with user space utilities such as
the port of FreeBSD's mfiutil(8) that I'm working on. I'm also including
some more structs and definitions that mfiutil(8) needs.

This required some rework of lmrc. Mostly it's just renaming a few
things, but there are a few spots where the code had to change a bit.

The same headers could be used by mr_sas, if anyone cared enough to give
it a similar rework.
