Yes, when I use Emacs 21.1 in MDL 8.2, there is no need to set the smtpmail-smtp-server, but in woody (Emacs 21.2), I have to set the additional variable. You do not need sendmail at all on your local host. Instead, smtpmail.el directly talks to a remote SMTP server.
