source, from openbsd's ksh.1:

	.Sm off
	.It *( Ar pattern\*(Ba No ...\*(Ba Ar pattern )
	.Sm on

groff and mandoc:
	*(pattern|...|pattern)

the only problem is that mandoc marks up the first \*(Ba in the same
font as "pattern", and groff does not.
