mdocml: Clean up warnings related to macros and nesting.

Clean up warnings related to macros and nesting.
* Hierarchical naming of enum mandocerr items.
* Improve the wording to make it comprehensible.
* Mention the offending macro.
* Garbage collect one chunk of ancient, long unreachable code.
