mdocml: Fix multiple aspects of SYNOPSIS .Nm formatting: * Don't break

Fix multiple aspects of SYNOPSIS .Nm formatting:
* Don't break lines before non-block .Nm elements.
* Use proper <b> markup for the heads of .Nm blocks.
* Make the width measurements work by doing them on the head children.
