[PATCH] Update contributor name

Hey,

Quick administrative patch here. Not contributed in a while, but I anticipate I may in the future, so let's get this in. This commit just updates a name in COPYRIGHT and adds a .mailmap file.

Note that in current git, for this to be reflected in your use of "git log" you will need to execute:
$ git config --global log.mailmap true
This setting will be the default in future versions of git, but is not the default now.
