After running with TeXLive 2008 for some time, I decided to try
the TeXLive 2011 dist and try to get 'caught up' with more
recent developments. I am on a Mac, and the MacTeX distribution
installed everything as expected in /usr/local. But things
do not work as expected.
With Mkii, doing 'texexec file.tex' on a simple file succeeds,
but the result file is a .dvi file, not a .pdf file as expected.
This is quite surprising, since the output indicates that pdftex
is being loaded:

For MkIV, I don't even get that far:

You should clean up your texmf-local directory, I suspect that that still has old files (from 2008?):
