Ah, that's the "aesthetics" I mentioned earlier: put the curly braces
around outer,high – that sends both values to the default:

\setupfloat
   [intermezzo]
   [leftmargindistance=-\outercombitotal,
    rightmargindistance=-\outercombitotal,
    default={outer,high}]

Delving further into details.pdf (I got this from a day of playing with
it--which is hardly adequate for really digesting one of Hans's manuals)
will probably yield even better results.
