Maybe I quoted the wrong part of the log, but it seems like the MkII
format is not generated:

  This is pdfTeX, Version 3.1415926-2.4-1.40.13 (TeX Live 2012)
   \write18 enabled.

  kpathsea: Running mktexfmt cont-en.fmt
  mktexfmt: No such file or directory
  I can't find the format file `cont-en.fmt'!
