Try the hanging environment:

    \setupexternalfigures [location=default]
    \starttext
      \starthanging{\externalfigure [cow.pdf][width=3.141cm]}
        \input ward
      \stophanging
    \stoptext

I think it is supposed to be the successor to hangaround.
