I’m still struggling with many [entry not flushed] in my indexes but 
couldn’t find a minimal example to reproduce the problem so far.

Most of the missing items are in tables (tabulates in the running text 
as well as split TABLEs in placetables), but not all.

It doesn’t happen if interaction is off*, while it doesn’t seem to 
depend on special index stuff like processors.
So I guess it’s still related to the tagging of links that caused the 
problems in the thread linked above.

Since the bookmarks are also interaction features, that problem may be 
related.

*) That means I can at least complete the print version of my book, phew!
