I'm not on the pdftex list, but I was just looking through their
archives and came across this document on the PDF 1.5 spec:

The most notable point of interest to me is on the extensions to the
tagged PDF spec (particularly starting on 10.7.4). PDF will now
support structured markup for pretty much all of the common structures
in ConTeXt, even citations!
Any chance we'll finally see pdftex (and later ConTeXt) support this
