Well, it's not yet a standard feature. A few month ago i made a hack for
spqr:  

\let\normalshipout\shipout
\def\shipout     {\afterassignment\funnyshipout\setbox0}
\def\funnyshipout{\aftergroup\veryfunnyshipout}

\def\veryfunnyshipout%
  {\ifdim\wd0>\ht0\relax
     \dimen0=\pdfpagewidth
     \pdfpagewidth=\pdfpageheight
     \pdfpageheight=\dimen0
     \normalshipout\hbox
       {\pdfliteral{q 0 1 -1 0 0 0 cm}\box0\pdfliteral{Q}}%
   \else
     \normalshipout\box0%
   \fi}

No guarantees.
