When I do \startitemize in a table cell, the items are all on one
line. How can I force newlines between them?

Normally a table cell is an hbox. You get an vbox when you use an p
entry.
\starttabulate[|l|p|]
\NC test \NC \startitemize[packed] \item test \item test \stopitemize
\NC \NR
\stoptabulate
should work ok.
