When setting up a layout, the main area of concern is 

  header+text+footer

this makes up the height. 

This area is positioned in the page by topspace and backspace (something
bindingedge). I admit that some english keywords are less typo than some
dutch ones, but alas.  

The width is the width of the typeset area. One can at all points ask
for the \textwidth and the \makeupwidth, these are normally the same,
but when typesetting in 10 columns, the first is is much smaller than
the second.

It sort of reflects that you are doing a manual makeup. It's one page,
actually a vbox similar to the text area. This is often used when making
a title page.
