We now do a lot of testing at kluwer (where we have a lot of input
examples), and we found a lot more bugs on top of the ones in your
list. (Yes, you need the \bblreadfalse under some conditions).
There will be a new version 'soon' with all of these bugs and more
fixed, and with better documentation on writing the styles.
