The header and footer problem is because you have put the
\noheadersandfooters command inside of the \vbox. Do it like
this, and it works:


  \startsetups [chapter:before]
     \page[left]
     \vbox to 35mm{}
     \noheaderandfooterlines
     \vbox{\blank
