piping syntax:
\input "|ruby ./myscript.rb \vartest XXXXXXXXX"
the magic trick is the | symbol. With your current tex:
\writ18{ruby ./myscript.rb \vartest XXXXXXXXX > whatever.tex}
\input whatever % or something using \openin ...
