I don't see why documents created by context/luatex should pose a problem with respect to adding annotations. If 38 annotations are good for 100M extra data then (1) the annotations are huge, or (2) the editing app messes up or (3) the user could try to 'save as' instead of 'save' (often forces cleanup) The kind of pdf produced by context/luatex is pretty simple and efficient and does not have any clever tricks. The auto magic is unrelated to the pdf.
