I suggest to use MkIV instead of MkII because it makes it easier to select a font.
\mainlanguage[ja]
\usemodule[simplefonts]
\setmainfont[Meiryo] % replace "Meiryo" with a font available on your system
\setscript[nihongo]
\starttext
Hello, World!
世界、今日は!
\stoptext
