the problem is that sometimes it's hard to deduce the state so i've added
\righttolefthbox \rtlhbox
\lefttorighthbox \ltrhbox
\righttoleftvbox \rtlvbox

\lefttorightvtop \ltrvtop

some cases are (using the default parser, which is a one-pass forward
scanner) tricky to determine
