Hello Gerard.

Moving the [offset=*] down to \startMPpage and eliminating the \setup MPpage worked here. (I also scaled it up so I could see it on my viewer.)

\startuseMPgraphic{test}

fill fullsquare rotated 45 scaled 20;

\stopuseMPgraphic

\starttext

\startMPpage[offset=5pt]

\includeMPgraphic{test}

\stopMPpage

\stoptext


This may be a feature.
