I am having the same problem trying to make an envelope (the form is for address). When I print the page, the forms comes empty. Anyone has some hints? Or is the only use to send the filled out form with the help of javascript to a webpage/script? I have tried a simple example (thanks Lutz) and the uptodate 1. All things that are inside a textline for example are not in the postscript output of the page ... I have also tried to use the standard postscript fonts -- but no success.
