Again, use textbackground. The following honors the present width


\setuplayout[middle]
\setupcolors[state=start]
\definetextbackground[screen]
    [backgrond=screen,
     frame=on,
     strut=yes,
     width=local,
     location=paragraph]

\starttext
   Here is an example,

\startitemize[n]
   \item \input tufte
    \startscreen
      Some background test
    \stopscreen
