I have done the essentially same for SGI Irix and linux (at home), and
also installed pdftex-1.11b from source and a newer cont-all.zip.  I do
keep my existing texmf-local/context/config/texexe.ini.  I can't reproduce
the problem on Irix using acroread 4.

d-en-all works here (except for the missing graphics):

Thinking the problem might be with the 2003.12.18 version of ConTeXt,
I installed http://www.pragma-ade.com/context/cont-tmf.zip and rebuilt the
formats:

ConTeXt  ver: 2003.12.18  fmt: 2003.12.31  int: english  mes: english

I also checked the default texexec.ini:

$ mv  ./context/config/texexec.rme  ./context/config/texexec.ini
$ texhash .
texhash: Updating /user/gwhite/texmf/./ls-R...
texhash: Done.

The next step would be to compare "texexec --verbose" for the two
configurations, and also the contents of the .tuo and .tui files.
