left=Q works. The label is

<text><space><left><number><stopper><right>, so this works

\defineenumeration
         [question]
         [text=Question,
          left=Q]

\starttext
      \startquestion[abc]
      Why are there so many spaces in \in[abc]?
      \stopquestion
\stoptext
