true from almost ten years
see page 37 of pdf cont-enp.pdf  3.3 page composition (the page of
pdf, not those printed on document)

true and it's ok, because you are using the default
\setuppapersize[A4][A4]
Try
\setuppapersize[A4][A3]
\setuplayout[location=middle]
\showlayout
\starttext
\input knuth
\stoptext
