Joining late to this conversation. I appreciate the size of the challenge discussed, but I for one would love it if we just got unique styles for bulleted and numbered lists so these styles could be customised using the reference-docx switch.
My requirement is for conversion from markdown to Docx and I need to have "- bullet" in markdown converted to a customised style in the docx file.
I know the OP was hoping for more, but I'd take this small improvement in a heartbeat if offered.

Not yet. My initial reaction is that, in order to handle all the possible combinations, we'd need a huge number of individual styles. Not sure I want to go there. Too bad Word doesn't have "cascading" styles.
