* Custom Footnotes in HTML

Consider the minimal working example below:

    $ pandoc --from markdown+footnotes --to html --wrap=none <<EOF
    Lorem ipsum[^1].

    [^1]: Here is the footnote.
    EOF

It produces

    <aside id="footnotes" class="footnotes footnotes-end-of-document" 
role="doc-endnotes">
    <hr />
    <ol>
    <li id="fn1"><p>Here is the footnote.<a href="#fnref1" 
class="footnote-back" role="doc-backlink">↩︎</a></p></li>
    </ol>
    </aside>

How can I customise the footnote writer to

   1. No have the <hr /> 
   2. Include a heading before the <ol>

Your best bet would be to postprocess the HTML.
