Including something *before* the header?

Use case: \usepackage{imakeidx} needs to appear before \usepackage{hyperref}

I’d prefer to not edit the template, but AFAICT, that’s the only way to 
solve this issue, right?

That way, my version of the template won’t grow stale when pandoc updates 
its default template in the future.

Ah, but you can fork the pandoc templates repository on GitHub, make
changes in your fork and then merge in changes from upstream when new
versions of Pandoc are released. I avoid merge conflicts by viewing the
upstream version side by side with my version in vimdiff and copying things
by hand. I first fetch upstream, then check out the upstream file in a
fresh branch, make the manual merge and I then merge into the main branch
automatically. Vimdiff is very helpful as it color codes the differences.
Since for me this mainly concerns the latex template it is quite manageable
even though I also have some special-purpose templates. It seldom takes
more than fifteen minutes.
