You can use wkhtmltopdf to produce PDF with pandoc with the options --pdf-engine=wkhtmltopdf --to=html -o mydoc.pdf https://pandoc.org/MANUAL.html#option--pdf-engine https://pandoc.org/MANUAL.html#variables-for-wkhtmltopdf The question is how well you would fare to get the forms source into the Markdown source except with (huge) raw HTML blocks.
