I'm batch converting old .doc files (files from 2007 created in Word 2011.
OS X) ready for conversion to Markdown.
However, during conversion the heading styles are lost (simply replaced
text size changes) so become useless for subsequent conversion to .md files.
How can I ensure the .doc to .docx conversion will preserve the heading
structure?

What are you using to convert .doc to .docx? This
seems more a question for that software than for pandoc.
