Yes common practice is to prefix patches with an Upstream: and Source: header, e.g.

Upstream: yes/no
Source: <link to upstream commit/patch/request>
---
--- a/foo
+++ b/foo
...
```

If it has not been submitted upstream, best to explain why, or submit it yourself. I assume this patch would be acceptable upstream.
