Of course, not only the cursor-up binding is missing from vi insert mode
in zsh 4 - the rest of the cursor bindings also seem to be missing.

Zsh 3.0.8 had:
$ bindkey
...
"^[[A"  up-line-or-history
"^[[B"  down-line-or-history
"^[[C"  forward-char
"^[[D"  backward-char
...
