I have now finished.
My model is:
AMD K6/233MHz
family=5
model=6
and Forsyth's value works fine.

I've put the revised instructions on the web site in the file 9/pc/amdcpu.
No reports from K5 or K6 3D owners yet (forsyth has a K5 but it's in a fileserver
that's been up for over a year and he's reluctant to take it down for this...).
