If I enter my INBOX and read the one unread mail there and then do

C-u 40 /o

The amount I get varies from 3 or 4 to 10 to 20. Seemingly
random. Something to do with article numbers? Can I re-sort them if thats
the case?

Siiliarly if I focus on the group INBOX (which has no unread articles)
in the group buffer and hit enter it prompts me for how many to fetch. If
I put in 1 it jumps to the next group with no unread articles. If I put
in 30 I get about 13 or so.

Yes, `C-u 40 RET' selects the 40 highest-numbered articles.  In groups
like INBOX that's usually a lot more than are actually there.

There's no easy way to fix that.

No.

Well, you can move all the articles from that group to itself, which
on most IMAP servers will result in consecutive numbers being given to
those articles.
