After hitting C-c C-a and using filename completion to enter the
filename, I routinely end up with a foo.tex file, where I wanted to
attach the foo.pdf file instead. No doubt, this is because pdf is
mentioned in completion-ignored-extensions.
But it seems that this value for completion-ignored-extensions is not
suitable in this particular case. Of course, when doing C-x C-f,
having ps and pdf and dvi in completion-ignored-extensions is a good
thing.
Any suggestions?

I agree. I've now bound `completion-ignored-extensions' to nil
before calling `read-file-name'.
