The number of questions related to SMTP finally broke the critical
mass to warrant a manual to be written, so I, err, wrote it.  It is
included in Emacs CVS and XEmacs mail-lib CVS (smtpmail.info) but also
available online:

http://josefsson.org/smtpmail.pdf

Note that some features (e.g., authentication, server workarounds,
debugging) may not be implemented in Emacs 20 or Emacs 21.x, but
otherwise the manual should apply to older versions too.  XEmacs users
can upgrade to the latest mail-lib package and they see all features.

Of course, comments or suggestions are appreciated.
